PROVIDER Plus+ is the online Provider Directory for Health Partners participating providers and members. The information is available in English and Spanish.
Click below to access our network of PCPs, specialists, hospitals, DME, and other ancillary providers according to the parameters of your search (i.e. for referrals for Health Partners members or a provider near your patient’s home).
Please keep in mind:
- Only participating providers may provide specialist services without prior plan approval. Health Partners members must have a referral to see a specialist (except for routine dental, vision or OB/GYN care).
- Health Partners members who need mental health or substance abuse care should be referred to their county’s HealthChoices behavioral health organization.
